Attention patients 

If you currently have any of the insurances listed below, we will no longer be able to continue to provide care to you under your current plan after November 1, 2024. We recommend contacting your insurance company for a list of providers that are in participation with your plan. 

• Medicaid
• Amerigroup - Medicaid
• Peach State - Medicaid
• Care Source - Medicaid

If you are affected by this change, we will be working with you leading up to November 1st, 2024 to transition your care.  Please check with your practice assistant at your next appointment or contact the office at 678-263-3080 for any additional questions or concerns regarding this transition.
